Varicocele surgery is Not a cure [1]. It is a high risk and low-benefit treatment method for varicocele [2, 3, 4].

"Your only treatment options are 'Varicocele Surgery' or to 'Do Nothing'; wait and see if your symptoms will worsen." Sounds familiar? This is the unfortunate advice that many clinicians are giving to their patients. But, they are both practically the same advice, because varicocele is a progressive disease, and varicocele grade and symptom severity often increase with age. So, may as well only say "Surgery is your only option."

If you have a really good doctor, he might recommend a scrotal supporter for you (which is also setting you up for failure).

Don't wait to see what will happen. Again, varicocele is a progressive disease. Waiting without treatment is a big mistake, because your varicocele grade and symptoms are likely to worsen with time. So, waiting and doing nothing is simply waiting to have surgery.

Taking pain killers and some anti-inflammatory medications might provide temporary relief, but these medications often come with the added side-effect of damaging stomach and bowel health, both of which are risk factors for varicocele. So, what might provide you temporary relief is also likely to worsen your symptoms.

Is surgery better or embolization? Is embolization a good "non-surgical alternative" to varicocele surgery? I mean surgery is high risk and embolization just makes a "small incision" right? Wrong. Not by itself. Varicocele normally worsens with time. As a matter of fact, the prevalence of varicocele raises by about 10% for each decade of life, reaching 75% in the eight decade of life. [1] Both varicocele prevalence and symptom severity increase with age! [2] So, the answer is: No, varicocele does not go away by itself, but actually worsens.

So, to prevent progression and worsening of the disease, it is critical to treat varicocele as soon as possible.

Surgery is not a good treatment option for varicocele, because it is both high risk [3] and likely ineffective [3, 4, 5]. The NNT for surgery is 17.[6] This means that out of 17 men who have surgery, only 1 will attain a successful pregnancy.

In order to repair a varicocele the following criteria should be fulfilled [2]:

1. Involuntary infertility of at least one year.
2. Presence of a palpable varicocele at the upright position and during a Valsalva maneuver.
3. No detectable or at least no irreversible cause of female infertility.
4. Normal testicular size (> 15 cm3) or small reduction of the ipsilateral testis [3].
5. Evaluation of at least two semen analyses at three months interval. The presence of normal semen analysis excludes varicocele as the cause of infertility. The usual effects of varicocele on sperm parameters include mild and moderate asthenospermia, teratospermia or astheno- teratospermia. Initially, sperm concentration is not seriously affected, though later all three semen parameters can gradually deteriorate, resulting in azoospermia in very few cases [4].
6. Normal values or slight increase (less than the double of the upper normal range) f serum FSH levels, as very high levels of serum FSH denote primary testicular failure.

Additional factors that must be considered before deciding the therapeutic approach of varicocele in an infertile couple include [5]:

1. The advanced age of the wife (older than 35 years) and high serum FSH levels should drive the decision towards the ICSI solution rather than repair of varicocele [6].
2. In case of chronic presence of varicocele and advanced male age, surgery should be avoided.
3. Surgical repair of varicocele is recommended in case of secondary male infertility [7, 8]. On the other hand, if the patient has primary infertility, azoospermia, small testicular size and high serum FSH levels, the presence of varicocele should be ignored and surgery should be avoided, as the diagnosis in this case pleads towards primary testicular failure.
4. The diagnosis of SCOS or maturation arrest in FNA or open testicular biopsy denotes primary testicular failure. Thus, the presence of varicocele should be ignored. On the contrary, the presence of mild or moderate hypospermatogenesis can be attributed to varicocele in which case surgery can be a reasonable therapeutic approach.

When I started, there was no alternative to varicocele surgery.  But, a safe and natural alternative needs to exist. So, I have been working diligently for the past 5 years to make natural treatments more of a viable option for everyone.
