Commando / Underwearless: Does it Help?
I went Underwearless and it helped, BUT...
You have to realize that there are 2 major heat cooling mechanism for the testicles. Remember--2!
Cooling Matters... A LOT: Your testicles have enzymes that only function at lower temperatures! Over-heating by just 2-3 degrees can completely halt sperm and testosterone production!
Scrotal Sagging: The first one we all know: Your testicles drop when it's hot to cool down by distancing themselves from your body. You're probably thinking: "Thanks for the big insight Genius..." But, realize that scrotal sagging is NOT the most important factor in testicular cooling, Countercurrent Heat Exchange is!
Countercurrent Heat Exchange: Now, this is the cooling mechanism of the testicles, and no one knows about it! The pampiniform plexus of the testicles is a meshwork of veins that cool the arterial blood that's going from the body to the testicles through a countercurrent heat exchange mechanism.
Now, as in the case of varicocele, what do you suppose happens when these veins become enlarged and blood does not circulate well within them? Well, you get almost complete disruption of the testicle's heat regulation!
So What? Can I Go Commando or Not?
Give me a moment! I'm getting to it!
So, varicocele disrupts the countercurrent heat exchange, and the testicles sag down to cool instead.
Yes, going commando helps, because it majorly the amount of heat that gets trapped. Minimal trapped heat is critical for testicular health and function. For men with varicocele, no underwear is typically better than normal underwear, simply because of less trapped heat. But, don't forget about the most important cooling mechanism!
Why Scrotal Support Underwear Works!
Men with varicoceles Need to wear proper scrotal support underwear. Why? Exactly what I've been getting at this whole post! You need to provide proper support and light compression so that the pooled blood in your testicles veins circulates! Proper counter current heat exchange and testicular blood drainage are far more important (for both testicular cooling and health) than allowing scrotal sagging.
So, Going Underwearless Helps, BUT... Proper testicle support underwear is better!
Key: Maximum Support & Cooling
It's time to throw out those heat trapping testicle death traps! Throw out your boxers. Throw out your briefs! But, don't just go out without any underwear... You need proper testicle support underwear that also maximizes cooling! To be exact, you need STUD Briefs: New underwear, intelligently designed with your testicles in mind.

7 Testicle Tips: How to Keep Your Testicles Cool!
Cooling & Support: Underwear Matters!
Look guys, The Underwear Your Wear Matters. It Matters A Lot! We are talking about Testicular Health. Your testicles are responsible for your fertility and testosterone production. Screw steroids! Get better underwear!
So, what is the best underwear for your testicles, to keep them cool and healthy, and provide the support they need to fight scrotal sagging? You'd be surprised that the typical cotton underwear doesn't make the cut.
Briefs or Boxerbriefs?
Definitely not! Briefs and boxer-briefs trap lots of heat! They are very bad for your testicles. You do Not want both compression and over-heating. Think about it: You compress and prevent your testicles from dropping to cool--and then you also overheat them? That's a recipe for cooked balls.
Conclusion: Avoid supportive underwear that has poor heat ventilation. Unless you want testicular damage.
Avoid Supporters And Jockstraps
Supporters and jockstraps are the worst! Not only do they typically trap a lot of heat, but they also typically over-compress the testicles. You really cannot find a good compression supporter because they all squish your testicles and penis together into a ball! Someone really didn't understand why your testicles are outside of your body when they designed these things.
Conclusion: Avoid both supporters and jockstraps whenever possible, unless you want impaired testosterone and fertility.
Boxers? Likely Not.
Boxers seem like a good option at first... I mean, they are more breathable right? Well, no, boxers trap just as much heat as briefs, because they typically have much more fabric! I have yet to find any boxers that are good at ventilating heat, and believe me, I've search through dozens of brands! Moreover, boxers allow scrotal sagging--which does not look good, and is unhealthy in the long run.
Conclusion: Boxers seem like a good option at first, but they are Not. Try to avoid boxers unless you want testicular overheating and scrotal sagging (which is unhealthy in the long-run).
Is Going Commando/Underwearless Better?
Going commando is probably the best option so far. I mean why are our balls--the one body part that needs to be kept cool wrapped up in multiple layers? It's irrational--an irrational social norm. Even though going underwearless is the best option for keeping your testicles cool, it still doesn't provide scrotal support.
Conclusion: Likely, by far the best option here--much better than briefs, supporters, and boxers. Going commando is a good option.
The Best Underwear: Cooling & Testicular Support
The best cooling underwear is one has a specially designed fabric, separates the penis and the balls, all at the same time providing light support for the testicles--improving blood circulation and not over-heating the testicles.
Conclusion: Buy the STUD Underwear it is the best cooling & testicular support underwear. It is great for varicocele.
